  • Visiting the office 313 W. Market St., Suite 4202, West Chester, PA. 19382.
  • Downloading the form by clicking on the appropriate link below: Forms must be printed, completed and returned to the Assessment Office by the appropriate filing date. No faxes or electronic copies please.
