What should I bring?

  1. Completed consent form (print and fill in form before you arrive)
  2. Health insurance card if you have one.

What will a Flu or COVID-19 vaccine cost?      

  • Flu and COVID-19 vaccine are provided at no cost. 
  • If you have insurance, please bring your insurance card with you to the clinic since participating private insurance plans can be billed.
  • If you do not have insurance, you will still receive a vaccine and will not pay any out-of-pocket costs.

What should I wear?

  • Shirt with short or loose-fitting sleeves.
  • Masks may be required at vaccination clinics. Please see your appointment reminder for more information. Limited masks will be available at our clinics. 

What Flu vaccines are available?

  • Regular 4-strain flu shots for people 6 months and older
  • Flublok egg-free flu shots for people 18 and older are currently unavailable due to a manufacturing issue. Please check back to see if they become available.
  • Fluzone High Dose flu shots for people 65 years and older
