This program is specially designed to assist adults age 55+ who care for children in their homes under age 18. Caregiver and child work with a specially trained care manager to design a care plan which meets the family’s care giving needs. Referrals may come from many sources, including schools, social service agencies, hospitals, and individuals.

Eligibility
- A grandparent, step-grandparent of a child, or a relative of a child by blood or marriage, who is:
- 55 years of age or older
- Is the primary caregiver of the child because the biological or adoptive parents are unable or unwilling to serve as the primary caregiver of the child
- Has a legal relationship to the child, such as legal custody or guardianship, or is raising the child informally
- Lives with the child
- The child must be under age 18
- The household income must not exceed 380% of the poverty level
Program Services
This program offers 5 basic services to caregivers:
- Aids caregivers in gaining access to services
- Individual counseling, organization of support groups, and training caregivers to assist them in making decisions and solving problems relating to their care giving roles
- Information to caregivers about available services for both the caregiver and the child
- Reimbursements for respite care to enable caregivers to be temporarily relieved from their care giving responsibilities
- Reimbursements for services or supplies, on a limited basis, to complement the care provided by caregivers
